Described by the Santa Clara Valley Water District as follows:
Brass disk in center of east side for concrete slab for telephone access box; 2 feet east from 6 feet tall telephone control box; located on west side of N Milpitas Boulevard; between Washington Street and Homme Way. City of San Jose.

Additional notes:
Although lately I've been identifying a lot of SCVWD benchmarks using their Vertical Control database, in this case I was in the area geocaching and this benchmark was located in approximately the same place as the geocache Peppertree Alley Redux.
