Full name of the church is Church of Saint John the Baptist and Transfiguration.
The parish was founded in 1911. It was located in a church on River Road in Rome, now home to St Michael's Ukrainian Catholic Church
The present Church was consecrated on 23 May, 1954. It was located in a predominantly Polish and Italian neighbourhood, but has evolved to serve all local Catholics.
The church features the Grotto of Our Blessed Lady near the front entrance. This grotto was at the original location on River Road and was bought from St Michael's for the 60th Anniversary in 2014.
