This Ontario Vertical Control disk is located on the south face of the southwest barrier wall of the Welland Vale Rd bridge over the Twelve Mile Creek. This appears to be a relatively new disk (monumented in 2020 if I understand the numbering system correctly) and it does not (yet) appear on the Ontario COSINE Online Service.
I might have missed finding this disk had it not been for the spray paint in the sidewalk. Pretty sure they did that so I would not miss it :)
Of historical note, the west end of this bridge sits directly on top of the remains of Lock 2 of the Second Welland Canal (built in 1842-45). You can see the stone wall of the former lock living the edge of Twelve Mile Creek.
