This is the Lyons VFW Aircraft display located on Route 14, south of Lyons, New York.

This aircraft is a Bell UH-1, version H, nicknamed IROQUOIS.
This helicopter was purchased by the US Army on Nov. 1967 and immediatly was shipped to Vietnam where from November of 1967 until October of 1971 it flew many missions, accumulating over 1,700 flight hours. It was brought back to the US Army base at St. Louis, Missouri late in the year 1971. After some time down for maintenance, it was loaned to the National Guard where it spent some time in Germany for two years from 1973 until 1975. In February of 1975 it was transported back to the US where for a time it was on loan to NASA. It finally found its permanent resting place in Lyons, New York on March 21, 1997 where it was mounted for display outside the Lyons VFW Post 5092. The Plaque located in front of the helicopter was printed on standard paper and placed inside a display case for viewing, however, moisture has caused the writing to become blurred and mostly unreadable. The facts listed above are what I could visibly decypher from the blurred writing on the plaque. I attempted to learn more about the aircraft through internet research, however, the serial number didn't come up on any of the recommended websites I searched.
