The NPS database and the website we use for reference both call this
Methodist Episcopal Church of Isle La Motte, a/k/a
United Methodist Church of Isle La Motte.
The brass plaque calls it Old Stone Church.
There is no doubt this is the correct structure, as there is nothing else at this address, not to mention the fact that it is the only church in town on the Register.
At any rate, the plaque goes on to describe that it was built by stonemason James Ritchie in 1843, and that it also served as Town Hall and a school until 1892.
