Galaxy Airlines Flight #203 crashed on take-off from Reno, NV on its return flight to Minneapolis from a gambling and skiing Super Bowl weekend.
The aircraft was an Lockheed Electra. The problem was that an access door was left open by the ground crew. The vibration upon take-off led to a chain of events in which the crew became confused and basically forgot to fly the airplane. This crash was one of the impetuses for the Cockpit Resource Management (CRM) training program which is required today.
This plaque is located on a rock along a walkway at the Rancho San Rafael Regional Park in Reno, Nevada. It rests among a grove of evergreen trees at the park.
A detailed story was filed by KARE 11 news reporter, Allen Costantini.
