Built in 1911-13 for prominent Salem physician and surgeon Dr. William Cusick, this home, due to it's extensive use of oak in the interior is also known as Oak Villa. It is a Colonial Revival style house designed by noted architect Frederick Legg. The house features a hipped roof with classical entablature and dormers.

William Cusick, along with his wife Marcia, were natives of Illinois who migrated to Oregon as children. He was one of the three graduates of the first class of the Willamette University Medical school in 1867. Cusick moved to Idaho but later relocated to Salem in 1882 where he practiced medicine until his death in 1919. Cusick was a Republican and served in the Oregon legislature after his election in 1884. After Cusick's death, his widow remained living at the house until her death in 1934.

In 1987, the house was a leading contender to serve as the Governor's mansion, ultimately finishing behind the Livesley House, located a few feet to the west.
